*For the I-9 supporting documentation, all documentation that requires a signature must be signed by the employee to be valid which includes passports and social security cards.
**On the State tax form, only choose either line 1 or line 2 to claim marital status of single or married. Both line 1 and 2 may not be completed on the form. Line 4 MUST be completed and should be either the sum of lines 1 and 3 or lines 2 and 3.
