- After completing the PEIA benefits enrollment online through the PEIA website, complete the basic life and optional life insurance beneficiary form on the MetLife website by clicking here. Please provide a copy of the PEIA benefit election confirmation to the HR Coordinator. MetLife should not be provided to HR or PEIA. All beneficiary elections and/or changes must be made online.

Annual leave accrual rate per pay period and maximum yearly carry forward amounts:

10-15 years 6.47 hours 35 days or 280 hours
Greater than 15 years 7.40 hours 40 days or 320 hours
